What to Expect

Your first visit starts with a consultation. We'll review your health history, take any needed imaging, and walk through your treatment plan and options together in plain language.

If you complete your registration form ahead of time, your visit can start with your consultation instead of paperwork. Please arrive 15 minutes early for your first visit.

Save time at your first visit by completing your registration form online beforehand. It covers your contact information, health history, and insurance details.

Complete Registration Form

Please also bring the following with you:

Your referral to the oral surgeon
A list of medications you are presently taking
Your dental insurance card or completed dental insurance forms

Insurance & Payment Options

We stay in-network with most medical and dental insurance plans, so cost isn't the reason you delay care. Our team will verify your benefits ahead of your visit and explain your coverage in plain terms.

For costs not covered by insurance, we offer flexible payment options. Ask our front desk team for details specific to your treatment plan.
